Registered Nurse (RN)
Department: Nursing

Position Summary
The Registered Nurse (RN) provides compassionate, resident-centered nursing care while leading and supporting the clinical team. The RN is responsible for assessing resident needs, administering medications and treatments, coordinating care, and ensuring the highest standards of quality, safety, and dignity for every resident.
Essential Responsibilities
- Assess, plan, implement, and evaluate resident care.
- Administer medications and treatments as prescribed.
- Supervise and support LPNs and CNAs.
- Monitor resident conditions and communicate changes to physicians and families.
- Maintain accurate medical documentation.
- Participate in care planning and interdisciplinary meetings.
- Ensure compliance with state and federal regulations.
- Provide leadership, mentorship, and clinical guidance.
Qualifications
Required
- Current Nebraska RN License.
- Graduate of an accredited nursing program.
- Current CPR certification.
- Strong communication and leadership skills.
Preferred
- One year of long-term care experience.
- Experience with electronic medical records.
License Practical Nurse (LPN)

Reports To: Director of Nursing

Position Summary
The Licensed Practical Nurse (LPN) provides quality nursing care while promoting the comfort, dignity, and well-being of residents. Working alongside the nursing team, the LPN administers medications, treatments, and assists in coordinating resident care.
Essential Responsibilities
- Administer medications and treatments.
- Monitor resident health and report changes.
- Assist with physician orders.
- Complete documentation accurately.
- Support CNAs in delivering quality care.
- Communicate with residents and families.
- Participate in care planning.
Qualifications
Required
- Current Nebraska LPN License.
- CPR certification.
- Excellent communication and organizational skills.
Preferred
- Long-term care experience.
- Electronic medical record experience.
Certified Nursing Assistant (CNA)
Department: Nursing
Reports To: Charge Nurse
Position Summary
The Certified Nursing Assistant (CNA) provides direct, hands-on care while helping residents maintain dignity, independence, and quality of life. CNAs play an essential role in creating a safe, compassionate, and supportive environment for those who call HMH home.
Essential Responsibilities
- Assist residents with activities of daily living.
- Answer call lights promptly.
- Assist with mobility and transfers.
- Observe and report changes in resident condition.
- Document care provided.
- Encourage resident independence.
- Maintain a clean and safe environment.
- Build meaningful relationships with residents.
Qualifications
Required
- Current Nebraska CNA Certification.
- CPR certification preferred.
- Compassionate and dependable.
Preferred
- Previous long-term care experience.
Dietary Aide
Department: Dietary
Reports To: Dietary Manager
Position Summary
The Dietary Aide supports the Dietary Services team by preparing, serving, and delivering nutritious meals while creating a positive dining experience for residents.
Essential Responsibilities
- Prepare and serve meals.
- Assist with food preparation.
- Deliver meal trays.
- Follow dietary restrictions.
- Clean and sanitize kitchen and dining areas.
- Wash dishes and equipment.
- Maintain food safety standards.
Qualifications
Required
- High school diploma or equivalent preferred.
- Strong customer service skills.
- Ability to work in a fast-paced environment.
Preferred
- Food service or healthcare dietary experience.
- Food Handler’s Permit, if applicable.
